Child and Teen Checkups Outreach

Child & Teen Checkups (C&TC) is the name for Minnesota’s Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Program. C&TC is a comprehensive health benefit for children and teens from newborn through the age of 20 who are enrolled in Medical Assistance.

Our goal is to keep children healthy!

Child and Teen Checkups include:

  • Information about good physical health.
  • Information about thoughts, feelings and relationships with others.
  • Questions and answers about your child’s health.
  • Complete physical exam
  • Immunizations (shots)
  • Hearing check
  • Vision check
  • Lab tests
  • Check on growth and development
  • Referral to the dentist

Use this health check up for Head Start, WIC, School or Daycare.
Be sure to bring all the forms you need with you to the Child and Teen Checkup.

Where do I get a Child and Teen Checkup?

You can get a Child or Teen Checkup from your doctor or clinic, or from some local public health departments – contact your local public health agency for details.

When should my child get a Child and Teen Checkup?

  • Between birth and 1 month old
  • At 2, 4, 6, 9, 12, 15, 18, 24 and 30 months
  • Every year after age 3 and up to age 20
  • Whenever sports physicals are needed
